In 400 words, or less, please tell us why your DSP nominee deserves national recognition. ANCOR is emphasizing criteria that successfully describe your DSP’s work ethic and the value they bring to the profession and those they support.
Suitable nominations must include information on each of the following criteria:
- Activity/accomplishments in building an effective social network, and meaningful and productive participation in the community for persons served (35 points)
- Description of the relationships he/she has developed with the person(s) he/she supports (15 points)
- Examples of advocacy or effecting change on behalf of people with disabilities (15 points)
- Description of the sacrifices made in order to be a Direct Support Professional (15 points)
- Creativity in individualizing supports and services provided (10 points)
- Demonstration of proficiency on the job and/or leadership capabilities (10 points)
- BONUS: Each nomination should also include an affirmation letter or statement from the person(s) served and/or a family member (10 bonus points)
