CMS Releases Health Homes for Enrollees with Chronic Conditions
CMS released November 16th preliminary guidance to states on the implementation of section 2703 or the Affordable Care Act. This section, titled “State Option to Provide Health Homes for Enrollees with Chronic Conditions.” This provision gives states the opportunity to address and receive additional Federal dollars for the coordination of primary, acute, behavioral health, and long term supports and services for individuals of all ages with chronic illness. The letter outlines CMS’s expectations for state’s successful implementation of the health home model of service delivery and provided initial guidance on important aspects of the health home provision.
