Under the Affordable Care Act, the Medicaid Health-Home State Plan Option allows states to develop health homes to coordinate the primary, acute, behavioral and long-term and social service needs for beneficiaries with chronic conditions.
To provide technical assistance to states, the Centers for Medicare and Medicaid Services (CMS) has established a new Integrated Care Resource Center website coordinated by Mathematica and the Center for Health Care Strategies.
The Integrated Care Resource Center has developed and gathered a variety of resources to share emerging best practices, including a new State-by-State Health Home State Plan Amendment Matrix that provides details from draft health home state plan amendments submitted to CMS. It also includes detailed information about target population, delivery system models, financing approaches, provider standards, care management features and many other program features to help guide additional states in program model development.
Two resources of particular interest to ANCOR members include the Five Key Considerations for Exploring the Medicaid Health Homes Opportunity and State Options for Integrating Physical and Behavioral Health Care.
