Administration Releases Request for Information on Person-Centered Care Planning for Multiple Chronic Conditions

The Agency for Health Care Research and Quality issued a Request for Information (RFI) seeking comments on the current state of care planning for people at risk for—or living with—multiple chronic conditions. The RFI requests feedback on issues including existing models of person-centered care planning, innovative models of care, and evidence for the effectiveness of strategies for implementation and delivery of person-centered care planning. Comments on the RFI must be received by November 15, 2022.
