NCAPPS Releases Person-Centered Practices Self-Assessment

The National Center on Advancing Person-Centered Practices and Systems (NCAPPS) released a self-assessment document “designed to help leadership at human service agencies in States, Tribes, and Territories to measure their progress in developing a more person-centered system. It is designed for use by the administrative entities (‘agencies’) with the authority to administer or operate federal programs for long-term services and supports, including home and community-based services. It can be used by the wide range of systems that oversee services and supports for people with disabilities of all ages, older adults with long-term service and support needs, and other health and social service programs.”
