You’ll Get
Hourly rate between $30.00 – 33.00 for LPNs
Hourly rate range of $35.00 – 38.00 for RNs

Qualifications

What You’ll Need
• To be a Colorado Registered Nurse (RN) or Licensed Practical Nurse (LPN) in good standing
• To successfully complete the provided required training
• Proficiency using Microsoft Office or other similar software
• Excellent written, verbal and listening communication skills (even when non-traditional communication is used)
• The ability to instruct and collaborate with others and to constructively receive and provide feedback
• To be detail-oriented with strong organization skills and the ability to independently manage time and prioritize demands
• The ability to respectfully address significant issues of health and safety while supporting an individual’s choices
• A valid driver’s license and reliable transportation is required

IF an LPN:
• 1+ years of experience working with the IDD (Intellectually and Developmentally Disabled) populations is required
• Working knowledge of the Colorado Development Disabilities systems, regulations and rules is required

IF an RN:
• 1+ years of experience working with the IDD (Intellectually and Developmentally Disabled) populations is preferred
• Knowledge of the Colorado Development Disabilities systems, regulations and rules is preferred

Physical Requirements and Working Conditions
Work is sometimes performed in private homes or in the community and occasionally in a typical office setting using a laptop and/or mobile devices. There may be frequent lifting, moving, and/or carrying of approximately 10 – 15 pounds, with possible bending, stooping, squatting, and walking and occasionally lifting, moving, or transferring up to 75 pounds.

Roles & Responsibilities

What You’ll Do
As a Nurse Case Manager, you’ll provide high-quality and inclusive healthcare and care coordination to customers with intellectual or developmental disabilities (IDD) on a caseload. You will develop, implement, and evaluate care plans in a community setting while working with other healthcare providers and agencies to integrate exceptional care.
Some of your responsibilities would be:
• Ensuring that services are provided in a manner that respects and acknowledges individuals’ self-expression, self-worth, self-reliance, and decision-making
• Developing and maintaining comprehensive care plans
• Training and informing customers, staff, and team members on health-related issues and topics in a comprehensive, understandable manner
• Promptly noting and addressing gaps in skills, understanding, or compliance with staff and contractors
• Communicating regularly with Program Directors, guardians, medical providers, and team members to ensure the customer’s health and wellness needs are met
• Maintaining knowledge and understanding of PPCH and other regulating agencies’ policies, procedures, rules, regulations, and guidelines
• Participating in the on-call rotation approximately one rotation per quarter
