Mosaic is looking for a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team!

In this role, you will provide health care to individuals receiving services in the homes by scheduling medical appointments, assessing condition, reviewing medications, completing any necessary medical forms and by updating medical records.

SCHEDULE: Monday-Friday, 8 hour shifts, 40/hrs. Weekend/Evenings on occasion. NO ONCALL – Positions are located in Grand Island and Beatrice, Nebraska.

Qualifications

What YOU’LL Need:
**State license required in good standing, ASN or BSN preferred or degree required based on state regulations.
**1 year of general nursing experience, if a new RN graduate previous LPN exp or new LPN graduate 3 years of CNA/Med Aide experience required – NEW GRADUATES WELCOME!
**Must be able to lift, push and pull at least 50 pounds.
**Valid Driver’s license

Roles & Responsibilities

What YOU’LL will do:
**Coordinate physician and health care services for people served
**Conduct health care assessments and write health care plans
**Facility training and orientation of new staff and med Aides regarding healthcare procedures
**Support the direct support or med aide staff with daily cares or medication administration as needed.
**Follow up with medical professionals for quality care of the individuals.
