This webinar is part 2 of the Data-Driven Decision-Making Web Series

The volume of data that is available for collection and use can intimidate staff and add steps, but also provides the opportunity for analysis that can offer key insights on opportunities for improvement.  In this second session, guest presenters will share their experience leveraging their program data to refine the management of care to improve outcomes for the people they support.  Looking at various healthcare and person-centered support metrics, especially during the time of this COVID-19 crisis, they have been able to identify opportunities for improving social determinants of health through alternative, less intrusive options. During this session, presenters will share examples of improvements they have implemented and offer best practices.

Kerry Delaney, JD is the CEO for Partners Health Plan (PHP). Prior to joining PHP, she served as the Acting Commissioner of New York’s Office for People With Developmental Disabilities (OPWDD). She also served as Executive Deputy Commissioner and General Counsel and was an Assistant Counsel to the Governor.
