Life transitions are stressful for anyone. And for many of us seeking to support people with mental health support needs and/or intellectual disabilities in their transition to community settings, we know that it’s not as simple as helping someone move from Point A to Point B.

So, with that understanding, how can we lean into therapeutic approaches for supporting someone to transition to a more integrated setting?
Join us for a session on utilizing positive behavior support and evidence-based practice to ease this transition. The session will also cover recommendations for developing effective multi-sector transition plans.
By attending, you’ll gain the skills to identify crucial considerations in transition planning, recognize specific support needs for people exiting forensic or criminal justice settings, and select relevant tools to utilize in planning.
