ACL Opens Applications for Community Care Hub National Learning Community

The U.S. Department of Health and Human Services Administration on Community Living (ACL) is soliciting applications to develop a Community Care Hub National Learning Community (NLC). ACL, with support from the Centers for Disease Control and Prevention (CDC), invites applicants to apply for the NLC to engage in collaborations aimed at furthering the capacity of Community Care Hubs nationwide.

ACL describes Community Care Hubs as community-focused entities that organize and support networks of community-based organizations in order to centralize administrative functions and operational infrastructure including: contracting with health care organizations, payment operations, management of referrals, service delivery fidelity and compliance, technology, information security, data collection, and reporting. The NCL will provide two tracks: a development track for organizations interested in serving as Community Care Hubs and an expansion track for established Community Care Hubs seeking to expand their capacities.
